Patient Checklist: HYPERPARATHYROIDISM EVALUATION
To ensure Dr. Harding can provide a complete and accurate assessment, please review and complete the following checklist before your appointment request is confirmed.
Dr. Harding must review the actual image files, not just the radiology reports. Patients are responsible for obtaining these images from the radiology facility and bringing them to their appointment. If left with our office, patients must also arrange to pick them up afterward.
Dr. Harding will perform a comprehensive in-office ultrasound as part of your evaluation.This may help reduce the need for additional imaging and lower your overall medical costs.
