In order to confirm an appointment, new patients will be requested to read and sign consent to AES and HIPAA policies that will be available online in the Patient Portal.

Submit Appointment Request
Patients request a preferred appointment time through the online appointment page under "Book Appointment". However, this request does not guarantee a confirmed appointment.
PLEASE NOTE:
Intake Process to receive a Confirmed Appointment Date and Time
No appointment is considered confirmed until the following steps are complete:
1. Patient intake process is complete
2. AES team has verified appropriate treatment
3. Dr. Harding is available at that time
4. Patient is sent confirmation of scheduled appointment
Self pay is required. Online Payment is accepted prior to confirmed appointment
Dr. Harding is an Out-of-Network (OON) provider. As such, all in-office consultations, visits, and related services are self-pay only and must be paid by the patient at the time of the appointment.
Insurance is not billed for office visits
Insurance is not accepted and will not cover nor be billed for in-office consultations, visits, and/or related services. Only specific surgical procedures are eligible for insurance billing and will be processed accordingly.
You can suggest a preferred date & time - but it may not be accepted
Submitting an appointment request through the online appointment page allows you to suggest a preferred date and time for your appointment.
HOWEVER, your appointment is not confirmed until you have followed the Intake Procedure to provide the requested information and have received a direct confirmation from our office via email.Submitting an appointment request does not guarantee a scheduled visit.
Your appointment is only confirmed once you have completed the Patient Intake process AND receive an email from our office with the approved date and time.
